ERISA Plan Documents

 

There are two types of benefit plans that are governed under The Employee Retirement and Income Security Act  (ERISA), employee welfare benefit plans and employee pension plans. The first category includes the following benefit plans at CUA: Health Insurance, Life and Accident Insurance, Long Term Disability, the Flexible Spending Plan and the Employee Assistance Program. The second category includes the Defined Contribution Retirement Plan as well as the Tax Deferred Annunity Retirement Plan (TDA Retirement Plan) .

There are some benefit plans at CUA that are not covered under ERISA. These "non-covered" plans include the tuition assistance plan and the tutition exchange program, as well as the metro check plan, among others. The documents below are copies of the legally required plan documents that have been created by CUA for certain benefit plans. If you are seeking a document that is not listed below please contact the CUA Benefits Manager at 319-4174.

For a copy of the new Retirement Plan Effective January 1, 2009 please contact Laurie Borman, Benefits Manager in Human Resources, at 4174. The new retirement plan was adopted pursuant to a Dec. 16, 2008 Board of Trustees Resolution. See the Summary of Changes to the Employee Retirement Plan. Note also that the two plans (TDA and Defined Contribution) have been combined into one plan document.
